Centuria Medical Makati Condominium Corporation (CMMCC) is committed to protecting the privacy of its data subjects and ensuring the safety and security of all personal data in our possession in compliance with Republic Act No. 10173, otherwise known as the Data Privacy Act of 2012 (the “DPA”), its Implementing Rules and Regulations (“IRR”), and other relevant issuances of the National Privacy Commission.

The Privacy Policy tells you how we process (meaning, how we collect, use, share, retain, and dispose and protect your personal data. It also tells you what steps you can take if you want us to correct how we use your data, or if you want us to stop using it.

In all instances, we assure you that processing your data will strictly follow the provisions of the DPA, especially with respect to general principles of data privacy in terms of Transparency, Legitimate Purpose, and Proportionality.

What personal data do we collect?

We collect the following information when you manually or electronically submit your inquiries or requests:

Why do we collect your personal data?

For documentation and processing of inquiries and requests within the Centuria Medical Makati, enable us to properly address them and forward them to the appropriate internal units for action and response.

How do we collect personal data?

We process your personal data on what you knowingly and voluntarily provide when you fill out through our website. The data you provide will be used to provide the service you requested and answer your queries.

To whom your data will be disclosed?

The management will ensure to maintain the confidentiality of data subjects’ information and are not allowed to disclose and share any information without your consent. Personal information under the custody of Centuria Medical Makati Condominium Corporation shall be disclosed and shared only according to a lawful purpose, and authorized recipients of such data.

How secure is your data?

We safeguard the confidentiality, integrity, and availability of your personal information by maintaining a combination of organizational, physical, and technical security measures based on generally accepted data privacy and information security standards.

Centuria Medical Makati ensures that adequate physical, technical, and organizational security measures are in place to protect personal information, confidentiality, integrity, and availability. However, this does not guarantee absolute protection against certain risks involving the processing of personal data such as when systems are exposed to targeted cyberattacks, malware, ransomware, and computer viruses or when manual records are accessed without authority.

However, adequate policies are in place to ensure appropriate security incident management in line with existing policies, circulars, and other issuances.

Where do we store your data?

We store files containing personal information on our computers and servers, which are kept in a secure environment. We may also store your personal information in our backup files. We ensure that proper measures are adopted to protect your information.

Personal data shall be stored in a database for (5) years after inquiries and requests are acted upon. After which, records shall be disposed of securely.

Other categories of data may be kept longer than (5) years when its retention period is determined by other relevant laws and regulations.

How do we dispose of your data?

Physical records shall be disposed of through shredding, while the digital files shall be anonymized, In all instances, our manner of disposal shall ensure that the personal information shall no longer be retrieved, processed, or accessed by unauthorized persons.

What is your right as data subjects?

Under the DPA, you have the right to be informed regarding processing the personal information we hold about you.

Further, you may be entitled to request:

  1. Access to personal data that we have processed about you. It is your right to obtain confirmation on whether or not data relating to you are processed;
  2. Rectification of your personal data. This is your right to have your personal data corrected if it is inaccurate or incomplete;
  3. Erasure or order blocking of your personal data whenever warranted;
  4. The right to object if the personal data processing involved is based on consent or legitimate interest;
  5. The right to data portability through which you may obtain and electronically move, copy, or transfer data securely for future use.

You may claim compensation if you believe that you have suffered damages due to inaccurate, incomplete, outdated, false or unlawfully obtained, or unauthorized use of your personal data or for violating your rights and freedoms as a data subject.

Suppose you think that your personal information has been misused, maliciously disclosed, or improperly disposed of or that your data privacy rights have been violated. In that case, you have a right to file a complaint with the NPC.

If there are changes in our Privacy Policy

From time to time, it may be necessary for Centuria Medical Makati to amend this Privacy Policy. If we change our Privacy Policy, we will post the revised version here, and will take effect immediately, so we suggest that you check here periodically for the most up-to-date version of our Privacy Policy. Rest assured, however, that any changes will not be retroactively applied and will not alter how we handle previously collected personal data without obtaining your consent unless required by law.
